Welcome to the STM Clinic Page! This page contains all of the info & forms you'll need to start your school year off successfully & safely.
The new school year is approaching, and hopefully, you all are having fun, staying safe, and soaking in (a moderate amount of) the sun! All praise to the Lord that we are able to return back to STM safely, and receive His goodness through our Catholic School Education!
We are excited to welcome all returning and new students to STM this 2022 - 2023 School Year. Please review the information below & submit forms before August 15th.
***Remember: If your children are new to the school OR entering PK4, K, or 7th grade, they will need an updated immunization record before entering the new school or grade***
1. Every student must submit a copy of their most recent immunization records to the school office before they can start school. Failure to comply with this requirement will result in your child being excluded from attendance at school until his/her immunization compliance is received and verified.
2. Every student must submit a Student TB Questionnaire which is reviewed by the school nurse. If the information provided indicates a need for further medical evaluation, you will be notified.
3. Every student must submit a NEW Student Health History form. It must be turned in by the first day of school. This is a requirement per the Archdiocese. ----> This form is new and updated from previous years!
4. There are new permission forms for Asthma, Diabetes, Seizure Disorders, Non-Specific Disorders (such as heart issues/ chronic kidney disease), Life-Threatening Food Allergies, and Medication Administration. Please discard any old health forms, they will not be accepted for the 2021-2022 Academic year.
5. All students who wish to try out for or participate in a school-sponsored sports team (soccer, volleyball, football, baseball, basketball, track & field, cheer team, developmental sports teams for 4th/5th graders) or afterschool sports camp (2nd - 5th grade) MUST have a Pre-Participation Physical Exam on file in the School Clinic. A new form is required for every school year. ***There is a FREE Archdiocese-sponsored Sports Physical event in August. 6. Students who require emergency medications to be stored at school must have these medications and their appropriate permissions in the school office by the first day of classes. Please review the guidelines for these medications in your STM Family Handbook and Help Center Page.
