St. Thomas More Parish School strives for excellence in the health and safety of our students, faculty, staff, parents, and visitors. We have a school clinic staffed by a registered nurse. Our goal is to maintain a safe environment for our students.
MEDICATIONS ALL medications (over-the-counter and prescribed) that are brought to school are required to have a completed “Medication Permission” form signed by both the parent and the prescribing physician, as well as an “Individualized Healthcare Plan” specific to the condition. Please find the appropriate forms under the Healthcare Plans link, along with the signed accompanying Medication Permission Form, which is required. Students who require emergency medications to be stored at school must have these medications and their appropriate permission slips in the school office by August 1st. This will allow for ample time to meet with parents and devise a personalized health plan for each student. Please review the guidelines for these medications in your STM Family Handbook.
IMMUNIZATIONS All required immunizations are to be up to date prior to the first day of classes per Texas State Law. We ask that a validated immunization record is turned in to the clinic prior to August 1st, so that we have sufficient time to verify the record and contact parents prior to the first day of school should additional immunization(s) be needed. An example of a proper validated immunization record can be found under the Immunization Information header. Please make note to contact your physician early during the summer in order to obtain an annual physical and an up-to-date immunization record in case your child was not seen since their last birthday for an annual exam. During your annual exam visit, please be aware that there are additional required immunizations for those students entering PK-4 & Kindergarten (DTaP, IPV, MMR, & Varicella), as well as the 7th grade (Meningococcal-MCV4 & Tdap) - as new vaccination requirements are necessary for attendance in those specific grade levels.
Failure to comply with the August 1st requirement could result in your child being excluded from attendance at school until his/her immunization compliance is received and verified.
*Remember: If your children are new to St Thomas More OR entering PK4, K, or 7th grade, they will need an updated immunization record before entering the new school or grade***
COMMUNICABLE DISEASE & EXCLUSION GUIDELINES Illnesses should be identified by the parent and the child should follow the recommendations and guidelines for exclusion criteria as stated in the STM- Family Handbook. We kindly ask that all students remain fever free without any fever-reducing medications 24 hours prior to returning to our campus. We also request that students not return to class until they have remained symptom free for 24 hours from vomitting and/or diarrhea. A fever is considered 100 degrees or higher. PARTICIPATION IN SCHOOL SPORTS All students who wish to try out for or participate in a school-sponsored sports team (soccer, volleyball, football, baseball, basketball, track & field, cheer team, developmental sports teams for 4th/5th graders) or afterschool sports camp (2nd - 5th grade) MUST have a Pre-Participation Physical Exam on file in the School Clinic, preferably by August 1st so that we can properly process the forms prior to the first day of practice. All physicals must be verified prior to the first practice. A new form is required for every school year. Please make sure to take note of the proper school year along the top of the form before you print. The newest Pre-Participation Physical Evaluation Sports Form can be found under the required forms header on this page.
**There is a FREE Archdiocese-sponsored Health Event: Free Sports Physicals- this event will be August 3rd at CHRISTUS- St. Mary's Clinic- 2002 South Wayside, Houston, TX 77023. Pre-Registration is required in July- https://archgh.swoogo.com/SportPhy .**
OTHER IMPORTANT INFORMATION *Everystudent must submit both the annual Student Emergency Information Form & Student TB Questionnaire which are reviewed by the school nurse. If the information provided indicates a need for further medical evaluation, you will be notified. *There are new Healthcare Plans related to: Asthma, Diabetes, Seizure Disorders, Non-Specific Disorders (such as heart issues / chronic kidney disease), Life-Threatening Food Allergies, and Medication Permission / Administration. Please discard any old dated health forms as they will not be accepted for the 2024-2025 Academic year.
